State-Funded Supported Living Services Program (State-SLS)
Ages 18 and older
Allows adults (18 and older) with an intellectual or developmental disability (I/DD) to temporarily receive services while waiting for enrollment in I/DD HCBS waiver programs or to receive ongoing supported living services in the person’s home or community when the person does not qualify for LTC Medicaid services.
Qualifying for the State-SLS Program
- Must be 18 years old or older
- Be a resident of the State of Colorado
- Have a disability as defined by the Social Security Administration (SSA)
**Eligibility for the State-SLS Program does not guarantee the availability of services and supports under this program

**State-SLS can also provide non-duplicative services to members that are not available in other HCBS-Medicaid waivers.
