Case Management

Foothills Gateway, Inc. is the Case Management Agency (CMA) for Larimer County.  As such, our primary role is to provide assistance to individuals and families with accessing services through state and federally-funded programs such as Medicaid Waivers.  We are here to help you or your family successfully navigate the long-term care system so that you can receive the supports you need.

Our Case Managers are dedicated to providing a person-centered experience where collaboration and listening to your needs and concerns are the priority.  Together, we hope to create a network of individualized supports that allow you to live the life you choose.
