Libby Nominated for Case Manager of the Year

We are thrilled to announce that Libby Hoy has been nominated for Case Manager of the Year! Each year, Alliance Colorado solicits nominations for their CM of the Year award to be announced during the IDD Awareness Day at the Capitol luncheon. This recognition highlights Libby’s exceptional dedication, tireless work ethic, and deep commitment to the individuals she serves. She is truly deserving of this honor, and her impact as a case manager is evident in everything she does.

Libby’s unwavering advocacy for her members and her ability to handle even the most challenging cases have made her an irreplaceable asset to our team. Her resourcefulness and creative problem-solving skills ensure that her members receive the support they need, even in the most difficult circumstances. She consistently goes above and beyond to secure resources and find innovative solutions, turning obstacles into opportunities for growth.

One area where Libby shines is her work with individuals who have lost their housing. She doesn’t just address immediate needs—she works diligently to ensure long-term stability for her members. By leveraging her extensive knowledge of available programs and services, she has helped many individuals secure new housing and connect with resources that support their personal goals, giving them a stable foundation to rebuild their futures.

Libby’s commitment extends beyond the individuals she serves. She is a natural leader who excels in team dynamics, bringing colleagues together with a shared focus on the well-being of those we support. Her outstanding communication skills foster collaboration, even in challenging situations, and she is always striving to align everyone toward a common vision of success.

In addition to her direct work with members, Libby is also an exceptional mentor and trainer. She plays a key role in onboarding new case managers, often stepping in to lead by example and guide new team members. Her leadership and compassionate approach extend beyond her own caseload, and she is a trusted resource for her colleagues.

Libby’s background in direct care, job coaching, day programs, and respite for residential services gives her a comprehensive understanding of the diverse needs of the people she supports. This well-rounded experience enables her to approach each case with a deep, multifaceted perspective, ensuring that every individual receives thoughtful and tailored support.

Perhaps most remarkable is Libby’s ability to handle tough conversations with care and empathy. She addresses difficult issues constructively, ensuring that all parties feel heard and respected. This skill, combined with her compassion and problem-solving abilities, is what makes Libby an outstanding case manager and a trusted member of our team.
