EARLY INTERVENTION SERVICE COORDINATOR
Position Closes: 04/04/2025
The Early Intervention Service Coordinator is responsible for planning, coordinating, and monitoring of services and supports provided to families of infants and toddlers eligible for Federally funded Part C services and State funded Early Intervention services. The EI Service Coordinator shall have an assigned caseload of individuals for whom he/she shall be responsible to advocate. The EI Service Coordinator will facilitate initial and ongoing Individualized Family Service Plan (IFSP) meetings, complete family assessment, support the family to ensure services are being delivered in accordance with the IFSP, monitor the provision of services, supports, and recommendations identified in the IFSP, assist in obtaining identified services and supports, maintain a master record and all necessary documentation, serve as the primary contact for obtaining and disseminating information, and facilitate and mediate conflict resolution. Inherent in all these functions shall be activities supporting families Rights and Procedural Safeguards under Part C, support, empowerment, and advocacy.

Requirements/Responsibilities:
- Bachelor’s degree required; or five years of experience in the field of developmental disabilities.
- Must be able to work some flexible hours and have own transportation.
- Must be sensitive to infant/toddler delays/disabilities and their needs.
- Must demonstrate respect for cultural and ability diversity.
- Must display a high degree of responsibility, initiative, interpersonal skills, and communicative abilities.
- Must have the ability to perform complex or varied tasks.
- Contact all assigned incoming families.
- Demonstrate ability to handle multiple assignments and competing priorities. Ability to multi-task while remaining highly organized in a fast-paced work environment required.
- Assist the family to identify and secure services and supports as outlined in the IFSP.
- Assist in maintaining the person’s eligibility for services and benefits as necessary and appropriate.
- Assure all follow-ups completed and documented.
- Driving is a required function of this job.

DIRECT SUPPORT PROVIDER – ADULT CARE SERVICES

The Adult Care Services program provides temporary respite care for adults with Intellectual and Developmental disabilities at our respite house in Fort Collins. Direct Support Providers have the rewarding opportunity to work with and support adults with activities of daily living, social and recreational activities. Must be able to attend trainings and meetings at the main facility. Required availability for shifts include weekdays, evenings, weekends, and overnights. Some intermittent schedule availabilities may be required. This is a great opportunity to provide excellent and meaningful care and supports to individuals with varying abilities and care needs & help make their time away from home a wonderful experience. Driving is a requirement for this position.
Duties and Responsibilities
- Ensure positive, safe environment for individuals receiving services.
- Complete daily household chores to ensure a clean and safe living environment.
- Monitor the health and safety of individuals, as well as keep current on individuals’ Health and Safety Plans
- Assist individuals with medication as need indicates.
- Read daily log before each shift and communicate with other staff members through daily log entries.
- Assist, support, promote, and train individual with independent life skills and a person-centered approach, including, but not limited to, menu planning and meal preparation, cooking with a recipe, and handling and storage of food.
- Assist individuals with personal care/hygiene when needed
- Manage crisis intervention as necessary (medical emergencies, behavioral situations, incident reports, etc.).
- Report all mistreatment, abuse, neglect, exploitation, other incidents and concerns to Program Director.
- Must have good communication skills and ability to work as a team member as well as independently.
- Communicate regularly with Program Director and other staff members; attend program meetings, staff development in-services, agency functions, and retreats.
- May be responsible for transporting an individual to and from ACS.
- Driving is a required function of this job and may include driving for community activities and other appointments.
-2 full time positions available at $18.75-$20.63/hr. DOE/DOQ, current CO DSP minimum wage for overnights
