Direct Services

Foothills Gateway provides direct service options to people living in Larimer County.  You can find descriptions of the service options we currently provide below.

 

Job Development/Placement Services:

Foothills Gateway’s Job Development and Placement Program can provide assessments, job development, and job placement. This program is for individuals aged 18 and older. Foothills Gateway, Inc. works with the Division of Vocational Rehabilitation (DVR) to provide these services.  FGI staff do community outreach to identify jobs and vocational assessment sites that can be used to support individuals with finding community integrated employment opportunities.  DVR authorizes and funds the services that Foothills Gateway, Inc. provides to individuals eligible for DVR services.

Transportation:

Transportation Services are provided for individuals aged 18 and older between their home and program or work settings. Depending on the needs of the person, transportation may be provided by FGI, program approved service providers, or through public conveyance such as taxis and the public transportation systems. Whenever possible and feasible, existing community transportation options are used.

Respite at Adult Care Services:

Foothills Gateway’s Adult Care Services (ACS) program provides short term relief from daily caregiving responsibilities in a home-like setting for individuals with intellectual or developmental disabilities (IDD), 18 years of age and older, and residing in Larimer County. Adult Care Services is open for respite services and care 24 hours a day, seven days a week, based on staffing availability.  ACS is closed on major holidays.  The program provides fun and meaningful activities, both on site and in the community, as well as care and support based on the individual’s interests, abilities, and specific care requirements.  Adult Care Services can serve as a Back Up Residential Services (BURS) for individuals living in host homes or with family caregivers.  Families are able to private pay for respite services based on a sliding fee scale and may also apply for Restricted Funds through the Program Director to help pay for scheduled respite services.

Crisis Response/START:

Foothills Gateway, Inc. provides an integrated and collaborative approach to assisting people with intellectual or developmental disabilities (IDD) in crisis.  This service includes consultative support and intervention, as well as intensive case management, wrap-around services with the goal of reducing the frequency, intensity, and duration of crisis events using the START model.  START is comprehensive model of service supports that optimizes independence, treatment and community living for individuals with IDD and behavioral health needs.  It promotes person centered approaches and training for individuals, families, and caregivers with a goal of improved diagnosis and treatment, effective services, decreased need for emergency services, and health and wellness for the individual with IDD and the caregiver. All Crisis Response/START Coordination supports are need-based and are provided at no cost to the person/family receiving the service.

Housing Choice Voucher Program:

Foothills Gateway, Inc. administers a Housing Choice Voucher (HCV) program which enables participants to afford community housing by subsidizing their rent with federal funds from the Department of Housing and Urban Development (HUD).  This program is open to individuals with disabilities in Larimer County.  The individual or family must meet published income limits and secure a unit within published payment standards, as well as meet other HUD requirements.
